You can write to your local NHS Area Team or Health Board. If you are not happy with our response, you can write you your local health board

Complaining on behalf of someone else:

Under the Data Protection Act, if you are complaining on behalf of someone else, we need to know that you have their permission to do so.  A letter signed by the person concerned will be needed to progress any adverse incident.

 

Complaining on behalf of a customer who has died:

You will need, in the first instance, obtain a copy of the death certificate and a letter from the executor of the estate to grant permission to look in to the issue.

If there is no will, the person requesting the information will need to provide a written documentation stating that they are acting as executor.
